Each year, Medicine Hat Community Housing Society (MHCHS), in its role as the Community Entity (CE) for the Government of Canada’s Reaching Home program, evaluates local progress through the Community Homelessness Report (CHR).
The CHR is a key planning and accountability tool that helps track community trends, measure outcomes, and identify the needs of people experiencing or at risk of homelessness. The report supports informed decision-making, strengthens coordinated access, and helps ensure local resources are directed where they are needed most.
Progress Through Data-Driven Action
Guided by real-time community data, including the By-Name List and Coordinated Access, Medicine Hat’s homelessness response continues to focus on prevention, reducing inflow into homelessness, and helping people secure permanent housing.
During the 2025-2026 fiscal year:
- 160 individuals were successfully housed in permanent housing.
- 107 individuals and households received housing loss prevention supports, helping them remain housed and avoid homelessness.
These outcomes demonstrate the community’s commitment to prevention, housing stability, and a more effective and coordinated system of care.
